An understanding of natural conception is important in order to understand
in-vitro-fertilization (IVF). Normally, a woman will produce a mature egg each
month. The egg (oocyte) is released from the ovary at the time of ovulation and
transported to the fallopian tube. Usually, it is in the fallopian tube that it
will encounter sperm and be
fertilized. The fertilized egg develops into an embryo that will travel to the
womb where it attaches and grows.
In IVF, the egg is collected directly from the ovary before ovulation and is
fertilized with sperm in the laboratory. The fertilized egg will be incubated
for a period of 3 to 5 days. The resulting embryo is then transferred into the
uterus (womb) passing through a small canal, called the cervix, which can be
accessed through the vagina.
There are also variations in the timing and method of transfer that may be
appropriate for certain patients. How is IVF performed? The following are
step-by-step descriptions of the IVF process.
To avoid an unexpected pregnancy while taking Lupron, it is very important to
abstain from intercourse or use protection the month before starting
medications. You can
discuss issues regarding abstinence with your physician.
In some patients, Lupron may be started on the second day of their period, to
take advantage of the initial release of FSH and LH, in a technique called
"Flare up". In both techniques, Lupron is continued daily in conjunction with
gonadotropins until the follicular recruitment phase is over.
The cycle of IVF treatment begins with the onset of the menstrual flow. Patients
attend the office for a baseline ultrasound and blood work. The ultrasound
evaluation ensures the absence of cysts in the ovaries, and the blood work
verifies low levels of the hormone estradiol. These results indicate to the
physician that Lupron has achieved its goal of down-regulation.

In vitro fertilization is fertilization of the egg by sperm that occurs outside
the body in a glass dish. In vitro means “in glass”, in Latin. This technique is
used if the sperm and egg cannot meet in the fallopian tube as a result of
blocked or absent tubes or, if the quality or quantity of the sperm is
insufficient to fertilize the egg. Originally, IVF was designed to treat women
with blocked tubes, but its use has expanded to include the treatment of
infertility resulting from of other conditions, such as endometriosis, male
factor infertility and unexplained infertility.
There are several different drugs and drug regimens used during IVF therapy.
Because each patients problems are unique, treatment is individualized and you
may find that your particular regime differs from that of others. This is normal
and reflects our goal of providing the right treatment for your particular
problem.
Before IVF is recommended, tests are performed to ensure that the ovaries are
capable of producing eggs and that the male’s sperm can fertilize them. When
ready to begin an IVF cycle, you will receive a medication that turns off
temporarily the normal monthly maturation of egg follicles in the ovary. After a
blood test ensures that this has been successful, a drug is administered that
stimulates the ovaries to produce more eggs than usual. The progress of this
stimulation is monitored by blood tests as well as ultrasound examinations of
the ovaries. When a sufficient number of eggs containing follicles has reached
the proper size, another medication, hCG, is administered to ripen the eggs and
prepare them for retrieval.
Approximately one and a half days after hCG administration, the eggs are ready
for retrieval. The egg retrieval is performed at the Outpatient Surgery Center
of the Weston Regional HealthPark, part of the North Broward Hospital District,
rather than in a physician's office. As a result, our patients receive a
comprehensive and exceptional care from prior to their procedures until after
they recover. Depending on the number of ripe follicles, the procedure takes
15-30 minutes. During the retrieval, you are medicated with Versed and Demerol
to achieve what is known as conscious sedation. The use of these medications
minimizes any discomfort you may experience and diminishes your recollection of
the procedure.

Male infertility patients receive diagnostic assessment through hormonal and
high-complexity semen evaluation in a state-of-the-art laboratory within the
practice. Sperm retrieval techniques (PESE, TESA, TESE) are effective treatments
for severe male infertility and are commonly performed in our facility.
For women, a previous sterilization by a tubal ligation does NOT have to be
permanent! Dr. Ripps is a Fellowship-trained and Board Certified (SREI, ACGE)
Reproductive Surgeon with more than 20 years of tubal ligation reversal (TLR)
experience. The procedure is usually completed on an outpatient basis and
patients are able to readily return to their routine activities.
This same-day procedure for women yields excellent surgical results with
re-opening of one or both tubes in 95% of cases. A pre-surgical consultation and
some testing help assure that TLR is the correct choice and that a successful
pregnancy is likely. Though the woman’s age is a factor, for women with
confirmation of open tubes, the
