The timing of the decision to proceed with an infertility evaluation varies from
couple to couple. While there are no “rules” regarding the length of time a
couple should attempt conception naturally, it is generally accepted that an
infertility evaluation should be performed when pregnancy fails to occur after
one year of unprotected intercourse. However an evaluation should be performed
prior to one year if a known male factor, such as undescended testicles or prior
cancer treatment, or female factor, including advanced reproductive age (over 35
years), exists.

In natural conception, the fallopian tube provides three functions: it picks up
the ovulated egg, it provides an environment in which fertilization can occur
and it transports the resulting embryo to the uterus for implantation. In cases
where the fallopian tube is not functional or other impediments to fertility are
present, the function of the fallopian tube can be replaced by in vitro
fertilization (IVF).
IVF is the most technologically advanced and successful form of fertility
therapy available. The pregnancy rates associated with IVF are the highest of
all fertility treatments across every age bracket. This is because IVF optimizes
all of the steps associated with the reproductive process. In a natural cycle,
only one egg is ovulated, fertilized and implanted in the uterus. IVF augments
these processes by stimulating multiple eggs to grow, fertilizing them directly
in the laboratory (in vitro) and placing multiple embryos into the uterus.

Robert Edwards, a Ph.D. physiologist, and Patrick Steptoe, a gynecologist,
pioneered IVF in Great Britain during the 1970's. Edwards had spent the 1960's
working with bits of human ovaries removed at surgery and had achieved the first
fertilization of a human egg outside the body in 1967. During these same years
Steptoe was helping to develop the new surgical technique of laparoscopy. By
1971 the two men had met and began to collaborate. Initially they retrieved eggs
from the ovaries of volunteers by laparoscopy and focused on improving the
timing of egg retrieval and in vitro culture conditions. By the mid seventies
they felt ready to attempt pregnancy. Their initial pregnancy was unfortunately
a tubal pregnancy (ectopic) in 1976. Then came true success and the first IVF
baby, Louise Brown, was born in July 1978.
