IVF-ET is probably the most well known of the Assisted Reproductive
Technologies. Otherwise known as "test tube baby," (fertilization actually takes
place in a dish and not a test tube) IVF-ET has helped many infertile couples
conceive and bear children for more than a decade. Originally IVF-ET was
developed to help couples overcome
infertility due to a problem with a woman's fallopian tubes. Now it has become a
useful treatment option with other factors such as immunological problems or
unexplained infertility.
First, you take medications to cause your ovaries to make multiple follicles.
This step is referred to as ovarian stimulation or super-ovulation. Second, the
growth of the follicles and development of the uterine lining is monitored by
the use of ultrasound. When it is determined that the follicles and the uterine
lining are appropriately mature, a trigger shot of Human Chorionic Gonadotropin
(hCG) is administered. The third step begins approximately thirty-six hours
after the trigger shot with the retrieval of eggs/oocytes. Guided by ultrasound,
the doctor aspirates the eggs from the follicles during a procedure performed in
the office.

Fertility Treatment Center
In Vitro Fertilization (IVF) and Embryo Transfer (ET) – are the most common ART
procedures. They are used to treat infertility problems caused by adhesions,
endometriosis, sperm antibodies, low sperm count, and unexplained infertility.
The eggs are retrieved and then processed by the laboratory. They are isolated
and mixed with properly processed sperm, and are then placed in an incubator
where fertilization takes place. After fertilization, embryos that have
developed satisfactorily
are transferred into the uterus three to five days after egg retrieval.
Intrauterine Insemination (IUI) - is also called artificial insemination. It is
a low tech form of fertility treatment. It is generally used in two ways:
natural and with ovulation
induction. Couples who are experiencing fertility problems may be able to use
IUI as a means to increase the likelihood of pregnancy. IUI is an in-office
procedure and uses a specially prepared semen sample that has been obtained from
the partner or donor. (You and your partner may elect to use donated sperm if
the male partner has no sperm or very poor quality sperm, if he has undergone
previous radiation or chemotherapy treatment, or if he has a genetic disorder
that might be inherited.) The sperm is placed gently in the uterus using a
catheter carefully maneuvered through the cervix. IUI is considered a relatively
painless procedure.
Semen Analysis - is a test that assesses the formation and maturity of sperm as
well as how the sperm interact with the seminal fluid. A fresh semen sample (no
more than a half hour old) is collected and then analyzed in a laboratory for a
variety of different factors. In order for sperm to be able to fertilize an egg,
it is necessary for seminal fluid to be of the correct consistency as well as
for sperm to have maximum motility and ideal morphology. If any of these factors
are revealed to be less than perfect in a semen analysis, male fertility may be
compromised. Male infertility testing is an important part of making an accurate
infertility diagnosis.

Intracytoplasmic sperm injection (ICSI) is a method of assisted reproduction
that involves combining an egg with sperm in a laboratory dish. It utilizes the
direct
microinjection of a single sperm into a single egg in order to assist
fertilization. If the egg fertilizes and begins cell division, the resulting
embryo is transferred into the
woman’s uterus where it will hopefully implant in the uterine lining and further
develop. IVF may be performed in conjunction with medications that stimulate the
ovaries to produce multiple eggs in order to increase the changes of successful
fertilization and implantation.
We want to make sure that your treatment has the highest probability for
success. Instead of providing one type of treatment for everyone, we will
perform testing so that we will treat your specific infertility. Blood tests
will tell us whether you have a hormonal problem and will help us to determine
the right amount of hormones you will need to take to produce the optimum
numbers of eggs. Some tests, like the sonohysterogram or the HSG, are done to
ensure that your uterus is normal and that no untreated problems exist –
problems that would make your treatment futile. Other tests, like trial
transfers are done so that prior to the actual treatment we know whether to
expect problems so that we can solve those problems before we are at the
critical stage of embryo transfer. A semen analysis is often performed to
determine whether the amount and quality of sperm are sufficient standard IVF or
whether we will need to use ICSI (sperm injection) to avoid possible
fertilization failure. We try to leave as little as possible to chance and we
will never perform unnecessary tests.
