IVF was the first procedure used to fertilize eggs outside a woman's body. In
1978, the first "test tube baby," was conceived through IVF. Most assisted
reproductive
technologies (ART) are derived from the IVF procedure.
The same hormones that stimulate one egg to be produced each cycle are given to
the woman in higher doses to produce multiple eggs in the ovary. These
medications are given at home by the woman herself, much the same way a diabetic
gives herself insulin injections.
During this stimulation process which takes about 10 days, the woman is seen in
the office 4-5 times for vaginal ultrasound assessment of follicle (egg) growth
and blood estrogen determination. These visits tell the doctor how many eggs are
being stimulated and when they are ready to be retrieved. A minimum number of
follicles must develop to make the retrieval worthwhile. Cancellation or
postponement of the cycle may occur if there are too few or too many follicles
noted on ultrasound.
Egg retrieval is performed while the woman is asleep under sedation provided by
Nurse Anesthetists. The doctor inserts a needle through the vagina into the
ovaries and into each follicle using ultrasound guidance. The follicle fluid
containing the egg from the follicle is aspirated into test tubes and given to
the embryologist for identification. Each egg is then placed in an incubator
where it will be placed with sperm a few hours after the retrieval. The number
of eggs retrieved can range from 0 (rarely) to 30 (rarely) depending on the age
of the woman and her ovarian sensitivity.
The egg retrieval takes approximately 20 minutes, and the woman then recovers in
the recovery room until she is ready to return home for a day of rest on the day
of egg retrieval.

Treatment with In Vitro Fertilization (IVF) involves several steps 1) the
development of follicles (pockets of fluid that contain the eggs) in the woman’s
ovaries- referred to as
“ovarian stimulation” 2) the removal of eggs (oocytes) from her ovaries-
referred to as “egg or oocyte retrieval” 3) The patient’s partner, if
applicable, will typically be required to collect a semen sample on the day of
oocyte retrieval. Exceptions to this include those patients using donor sperm,
those utilizing a previously frozen semen sample or those having surgical
retrieval of tissue or sperm extraction. 4) the fertilization of the eggs in the
laboratory- referred to as “fertilization of oocytes” 5) transfer of embryos
(fertilized eggs) into the (a) woman’s uterus- referred to as “embryo transfer.
Patients are not guaranteed success at any or all of these steps. If adequate
results are not achieved at any step, the cycle may be cancelled at the
discretion of an RMIA physician.
Preimplantation Genetic Screening (PGS) or Preimplantation Genetic Diagnosis (PGD)
is a process of screening early embryos during the in vitro process. The oocytes
are retrieved, fertilized using ICSI, and after three days of growth, biopsied
to obtain genetic material needed to screen the embryos. After this biopsy
occurs, the embryos are kept in the laboratory while the material is being
screened for a diagnosis. For information on disorders, screening, and/or

In the case of IVF with “Donor Egg” (or Oocyte), medications will be used to
coordinate the recipient patient’s cycle with the cycle of the egg donor. If
pregnancy occurs, the recipient patient will need to continue taking these
medications through the first trimester of pregnancy.
Following retrieval, the donor eggs and recipient partner’s sperm (or donor
sperm) will be processed in the laboratory to accomplish fertilization. Several
days later, the
fertilized eggs, will be transferred to the uterus of the recipient.
Embryo freezing/cryopreservation is a process where embryos (fertilized eggs) of
suitable quality are frozen in small vials using liquid nitrogen and specialized
equipment. The embryos can then potentially be thawed and transferred to the
uterus at a future date. The major benefit of embryo cryopreservation is to have
suitable embryos available for transfer without having to incur the costs and
time commitment associated with a fresh IVF cycle. In some cases, there may be
enough embryos that are cryopreserved to allow for several frozen embryo
transfer attempts. The main risk associated with embryo cryopreservation is
failure of the embryos to survive the freezing and thawing process.
