With special interest in ovarian function issues, specifically polycystic
ovarian syndrome (PCOS) and ovarian reserve testing, Dr. Daneshmand has
presented his research findings to colleagues from Los Angeles to New York. This
common condition of reproductive aged women is frequently misdiagnosed but Dr.
Daneshmand has helped both PCOS and other infertility patients achieve
parenthood. He also has published several papers addressing the decline in egg
quality that occurs with advancing age. He and his partner Dr. Bruce Shapiro
have been instrumental in the births of over 3,600 babies including some very
high-profile patients.
Adept at all aspects of both male and female infertility, Dr. Daneshmand
especially enjoys his interactions with patients and has earned a devoted
following of patients and is also a sought-after media spokesperson.
An Assistant Professor for the University of Nevada School of Medicine, Dr.
Daneshmand helps train medical students and residents. He is a frequent
contributor and reviewer for professional journals such as Fertility & Sterility
and Human Reproduction. Because of his research in ovarian issues, Dr.
Daneshmand helped establish the city’s first oocyte cryopreservation or egg
freezing program that affords women facing cancer treatment the opportunity to
preserve their biologic fertility for the future. The Center’s success in this
arena has shown great promise and the egg freezing is also available to women
who would like to preserve their fertility for the future but to circumvent the
decline in egg quality that occurs with advancing age.
Dr. Daneshmand is a member of the Alpha Omega Alpha honor society, and the
DeCherney Society and is held in high esteem in organizations such as Society
for
Reproductive Technologies and American Society for Reproductive Medicine.

In vitro fertilization (IVF) literally means "fertilization outside of the body"
compared to in vivo fertilization which is "fertilization within the body". IVF
has made it possible for
millions of infertile couples to create healthy genetically related children.
In a natural cycle, only one or two eggs usually develop. An IVF cycle requires
more eggs because some are invariably unhealthy or damaged during retrieval and
fertilization. Several embryos are also necessary, as some stop growing during
culturing. This is why additional follicle stimulating hormone (FSH) is
administered by
injection during an IVF cycle. FSH is naturally produced by the pituitary gland
and stimulates the recruitment and development of eggs within the ovarian
follicles.
In IVF, FSH is administered according to certain protocols usually in
combination with Lupron or Antagon. These fertility drugs cause the body to
dramatically reduce the production of leutinizing hormone (LH) which is the
hormone that signals ovulation. Ovulation must not occur before the eggs are
ready for retrieval and is triggered by a timed injection of HCG, which mimics
the actions of LH. IVF protocols determine the type of medication, dosing, time
of administration, duration, and other factors
The mature eggs are removed from the follicles using a small needle passed
through the back of the vagina. This process is guided by ultrasound and
performed under
anesthesia. As the eggs and follicular fluid are aspirated (retrieved), they are
immediately passed to the embryologist who isolates the eggs and transfers them
to a special media.
Some women cannot produce eggs that will fertilize and develop due to many
factors including premature ovarian failure or diminished ovarian reserve. In
these cases, an IVF donor egg can be used. Egg donors undergo the stimulation
and retrieval procedures and their eggs are combined with the partner's sperm.

The male supplies sperm by masturbation on the day of the retrieval. The sperm
is specially washed and prepared for exposure to the eggs. Once the eggs are
separated into the appropriate media, sperm are added and fertilization is
allowed to occur. Thousands of sperm surround the egg; however, once one sperm
has attached to and penetrated the eggs membrane (zona pellucida) others are
prevented from entering.

Dr. Littman is a member of the American Society of Reproductive Medicine (ASRM),

Dr. Littman is board certified in Obstetrics & Gynecology and is subspecialty
trained in Reproductive Endocrinology and Infertility. She is a skilled
practitioner in all aspects of infertility care including in vitro
fertilization, ovulation induction, laparoscopy and hysteroscopy. Dr. Littman
has also been involved in several medical mission trips to underserved areas of
third world countries where she has overseen cancer screening clinics and
general health maintenance for people with limited medical access.
