When the sperm count is very low ,then a single sperm is injected in to the egg.
Even if sperm are absent in ejaculate, then sperm can be extracted from testis (
Testicular Sperm extraction). it needs a micromanipulator with which an egg is
held by suction and with a sharp needle a single sperm is injected and the
fertilization is achieved. It is a boon for the couple where the routine IVF is
not successful because of failed fertilization by conventional IVF or where the
sperm count is very low or sperms are nonmotile.
The stimulation protocol for the ICSI is same as that of conventional IVF. Eggs
are surgically removed as in IVF . In normal circumstances the eggs are
surrounded by
cluster of cells called cumulus corona cells ,all of which must removed before
the sperm injection takes place. Removal of the cells gives an insight of the
eggs to
embryologist ,to assess the maturity of the egg . Sperms are collected from the
male partner , either by masturbation or they are surgically retrieved from
testis.
Denuded eggs are placed in a dish under 200 or 400 magnification with a inverted
microscope fitted with the micromanipulator .We have Nikon Narishige
combinations of ultimate quality . The eggs is around 100 micron in width and is
held by a glass holding pipette with a diameter of 40 to 50 micron usually on
the left side . Injection needle is heving a diameter of five to six micron.

ICSI-Quality Infertility Treatments
In order for pregnancy to occur, an egg has to be released from the ovary and
unite with a sperm. Normally this union, called fertilization, occurs within the
fallopian tube, which joins the uterus (womb) to the ovary. However, in IVF the
union occurs in a laboratory after eggs and sperm have been collected. Embryos
are then transferred to the uterus to continue growth.
The retrieval procedure to obtain the eggs is performed trans-vaginally using a
hollow needle guided by the ultrasound image (this is completely comfortable
under adequate sedation and local anesthesia). Eggs are gently removed from the
ovaries using the needle. This is called "follicular aspiration." Its timing is
crucial because the egg will not develop properly if it is collected too early;
if too late, the egg also may develop poorly or may have already been released
from the ovary and lost.
The eggs are immediately identified by our embryologists in our special IVF
laboratory. They are placed with the sperm in incubators to allow fertilization
to take place. The eggs are examined carefully at intervals to ensure that
fertilization and cell division have taken place; the fertilized eggs are now
called embryos.
Embryos are usually placed in the wife's uterus 2 or 3 days after egg retrieval.
A speculum is inserted into the vagina to expose the neck of the womb (cervix).
The embryos are suspended in a tiny drop of fluid and then very gently
introduced through a catheter into the womb, often under ultrasound guidance.
The transfer is followed by a resting period, blood tests and possibly
ultrasound examinations to verify if pregnancy has been established.

However there is a dark side to IVF as well. All the media hype about IVF has
created a lot of pressure on infertile couples. Most family members and friends
naively think that IVF represents a sure shot success method, and cannot
understand why infertile couples balk at going through IVF treatment. Moreover,
since the media only highlights the success stories, patients themselves often
have unrealistic expectations from IVF treatment. Unscrupulous clinics which
quote highly exaggerated and misleading pregnancy rates simply add to the
confusion.
Media coverage about infertility has also given birth to a new medical condition
I call “pre-infertility”. Preinfertile couples are usually young couples with no
medical problems, who start to panic because they hear their biological clocks
ticking. Even though they have no medical problems, and would have conceived in
their own bedroom if they had given themselves enough time, they often rush
headlong into seeking medical assistance . Unfortunately, many doctors are more
than happy to provide this even though it is not needed, because such patients
boost the clinic’s success rates and give the doctor an undeserved reputation of
being a good fertility specialist !
Because the treatment is so expensive, many doctors feel IVF is a lucrative
business proposition, as a result of which many IVF clinics start simply to make
money. They then pressurise infertile couples ( who could get pregnant with
simpler treatment options) to go in for IVF, leading to unnecessary
overtreatment. Since these are often young couples with minimal medical
problems, their pregnancy rates with IVF are high, thus boosting the clinic’s
success rate, and their reputation as well.
