In order to have the optimal outcome with your IVF treatment, we review additional medical information, perform pretreatment, and initiate stress reduction options. Lab work will include hormone levels, antibiotic treatment, and uterine assessment for additional documentation.
Our IVF Program (in vitro fertilization) is one of the most respected and successful programs in the United States, and it is clearly a leader in the Southern California region. Our seminal work in developing the technique of blastocyst culture and transfer led to our first "blastocyst" baby delivered in 1998. We have continued to improve and perfect this tremendous laboratory advance. We now offer the exciting technology of blastocyst transfer to all couples in our IVF program, when appropriate. Fertility specialists, Drs. Hummel and Kettel, have been involved in pioneering studies of pre-implantation genetic diagnosis (PGD) during IVF that can help recognize genetic disorders before embryos are transferred.

David B. Smotrich, M.D., F.A.C.O.G., is a Diplomate of the American Board of Obstetrics and Gynecology specializing in Reproductive Endocrinology and Fertility. Dr. Smotrich earned his M.D. degree from the Sackler School of Medicine, University of Tel Aviv, Tel Aviv, Israel. He completed his Post-Graduate Residency studies and Fellowship training in the Division of Reproductive Endocrinology and Fertility in the Department of Obstetrics and Gynecology at The George Washington University School of Medicine in Washington, D.C., and in the Department of Obstetrics and Gynecology at the Albany Medical Center Hospital in Albany, N.Y.
Dr. Smotrich has served as an Adjunct Scientist at the National Institutes of Health in Bethesda, MD, specializing in the care of women with hormonal abnormalities and premature ovarian failure. He was a member of the clinical teaching faculties at the George Washington University School of Medicine in the Division of Reproductive Endocrinology and Fertility and at the Albany Medical College.
In recognition of the sensitivity he has brought to the many medical and reproductive endocrinology students he has trained, as well as his tireless devotion and academic excellence, Dr. Smotrich was honored with the highly coveted Teacher of the Year award at the George Washington University School of Medicine. He also was honored with the highly esteemed Golden Apple Teaching award at the Albany Medical Center for his superior teaching skills.

The post-coital test (also known as a Huhner) is an excellent means of assessing the interaction of the sperm and the cervical mucus. In order to reach the fallopian tube and fertilize an egg therein, the sperm must first migrate through the cervical mucus. There are many factors which can impair the ability of the sperm to survive and traverse the cervical mucus, including infection, prior surgery on the cervix, and production of antibodies (substances that can kill or immobilize the sperm).
During a normal menstrual cycle, there are only a couple of days during which the sperm can survive in the cervical mucus. At other times of the cycle the mucus is a very effective barrier. Around the time of ovulation, the cervical mucus becomes very thin and watery and probably even somehow assists the sperm as they migrate through to the uterus and on to the fallopian tube. The quality of the cervical mucus as well as the number of sperm present and their motility will be assessed. The timing of this test is, therefore, crucial and must be done within 12 hours after Intercourse.
The post-coital test is an important part of the evaluation of difficulty conceiving. If it is not normal, other evaluation or therapy may be suggested.
A procedure by which a sample is taken of the endometrial lining of the uterus, shows evidence of ovulation and degree of maturation of the uterine lining, and can reveal uterine cancer, uterine fibroids, uterine polyps, and adenomyosis. This test also reveals if the woman has a luteal phase defect - a hormonal imbalance which prevents a woman from sustaining a pregnancy because not enough progesterone is produced. The test is typically performed 1-3 days before onset of woman's menstrual flow.
